   Case
 
A 58-year-old woman was referred to the hospital with a 2 months history of anorexia, fatigue, cutaneous nodular swellings, recently developed acute renal failure and hypercalcaemia. On admission, her blood pressure and pulse were normal and physical examination revealed (+) pretibial oedema and reddish nodular swellings on her face, elbows (Figure 1) and legs.
